This form when properly completed fulfills the requirements set out in the High Rise Communications Protocol between Altogether Group Pty Ltd, the Owners Corporation and their contracted Facilities/Building Manager (Protocol) to confirm the following on an annual basis and otherwise as reasonably required.

The responsibilities of the Owners Corporation include the following:
- Annual cross connection checks were completed covering no less than 20% of water End User connections across the buildings.
- Cross connection checks were undertaken by an appropriately qualified and trained individual as arranged by the Owners Corporation. [Note: Alternatively, and if agreed, Altogether may undertake these checks on the Owners Corporation’s behalf for a fee.]
- The Owners Corporation ensured that all plumbing works for End Users were completed by licensed plumbers in accordance with AS/NZS 3500 with Certificates of Compliance (CoC) supplied to Altogether.
- The Owners Corporation educates customers, end users, plumbing contractors and Facilities/Building Manager staff about the dual plumbing system and the risk of cross connections.
- Backflow prevention device testing results are provided to Altogether in accordance with the requirements outlined in the Protocol.
